HRT (hormone replacement therapy) is a safe way to replace those hormones and restore balance to your body. Every woman's experience of menopause is different—some women may have few or no symptoms; others may find menopause to be extremely challenging. But if you find that your symptoms are hard to manage or persist post-menopause, HRT may be right for you!

HRT has been shown to effectively counterbalance many of these unpleasant symptoms, allowing women to lead normal lives without having to suffer from discomfort and pain. In most cases, HRT is also recommended for women who begin menopause early—this includes women who start their final period before age 40 or whose periods stop before age 45. If you're going through premature menopause because your ovaries were surgically removed or naturally stop producing estrogen early in life (before age 51).

What Are Bioidentical Hormones and Pellet Therapy?

Hormone pellet therapy is an alternative to conventional hormone replacement therapy (HRT). It involves slow-releasing pellets that deliver bioidentical hormones directly into the body, reducing side effects and improving results.

Most female hormone replacement therapies are made from synthetic materials. Bioidentical hormones are derived from plants, like soy and wild yams, and more closely match the body's natural hormones. For instance, phytoestrogens found in plants are similar to the estrogen produced in the female body.

Hormone pellet therapy refers to the way the hormones are delivered to the body — in pellet form rather than a pill, cream, patch or injection.

Conventional HRT in pill form has been linked to blood clots, and long-term estrogen-only therapies can increase the risk of breast cancer in postmenopausal women. Bioidentical hormone replacement therapy (BHRT) in pellet form is considered a safer and more effective option for women suffering from a hormonal imbalance:

  • Reduces overall body fat and increases lean muscle, making weight management easier.
  • Maintains higher bone density, which can help prevent osteoporosis later in life.
  • Stabilizes your mood.
  • Increases your sex drive.

What Is Menopause?

Menopause is the time in a woman's life that marks the end of her menstrual cycle. It occurs when a woman's ovaries stop producing the hormones estrogen and progesterone. This happens naturally, usually after age 45, although the average age of menopause is 51. If you have not had a period for 12 months, you have likely reached menopause.

Before that happens, women go through perimenopause. Most symptoms occur during perimenopause when hormones are fluctuating wildly.

Some women go through early menopause and may experience these symptoms as young as 40. If you are between the ages of 45 and 55 and experiencing any of the following symptoms, have your doctor run a hormone test:

  • Hot flashes.
  • Night sweats.
  • Mood swings.
  • Heart palpitations.
  • Insomnia and sleep disturbances.
  • Fatigue.
  • Anxiety.
  • Forgetfulness and poor concentration.
  • Weight gain, especially around the belly.
  • Loss of breast fullness.
  • Hair loss.
  • Irregular periods.
  • Vaginal dryness.
  • Reduced sex drive.

How Does Hormone Pellet Therapy Work?

Pellets are the newest bioidentical hormone replacement therapy (BHRT) delivery method. They're tiny, they're convenient, and they're all natural.

As with any other type of hormone replacement therapy (HRT), pellets can be delivered in a variety of ways. You can choose them in pill form or as a cream that you rub onto your skin. But if you want to take advantage of the convenience and safety of a pellet, it's best to get it implanted under your skin. This is done in an office setting with local anesthesia, so you won't feel any pain at all!

Pellets are about the size of a grain of rice, so once inserted into your hip area (where most people have their pelvis), you won't even notice it's there. Your body slowly metabolizes the pellet and releases hormones into your system at a steady rate — just like nature intended. One pellet lasts between 6 months and 1 year depending on how fast or slow your body metabolizes it. If further treatment is needed, another pellet can be placed right next to it without any problems or side effects whatsoever!

Is Hormone Pellet Therapy Safe?

So, you've decided to try hormone pellet therapy. You may have heard some controversy about it—is it safe? What are the risks?

The most significant danger is with estrogen-only treatments, which put women at a higher risk of breast cancer and other conditions. At the biostation, we administer a combination of estrogen and progesterone that reduces that risk. Testosterone treatment lowers these risks even further, as no estrogen is delivered to the body.

Whichever treatment you choose, regular checkups are advised so that we can closely monitor your hormone levels to ensure they are appropriately balanced. Furthermore, the implanted pellets cause no harm to the body. The pellets are made of the actual hormone that is compounded and shaped into a small pellet. No other material is used to encase it.

Safety

Your safety comes first. Hormone therapy is considered when symptoms are bothersome and benefits outweigh risks for you. If you still have a uterus, estrogen must usually be paired with a progestogen to protect the uterine lining. If you’ve had a hysterectomy, estrogen alone may be used. We review your history (breast/uterine cancer, blood clots, stroke, heart disease, liver disease, migraines with aura, unexplained bleeding), medicines, and goals before making a plan. We also check blood pressure and discuss family history.

Pellet therapy uses small implants placed under the skin that release hormones over months. Because pellets are typically compounded (not FDA-approved products), dose consistency and safety data are more limited than with approved patches, gels, sprays, or pills. Pellets can’t be removed easily if the dose feels too strong. Local site issues (tenderness, bruising, infection, or rare extrusion) can occur. For these reasons, major medical groups recommend using FDA-approved formulations when possible and reserving compounded pellets for special situations. If a pellet route is chosen, we’ll set a careful follow-up plan and keep the dose as low as needed for symptom control.

Most healthy women who start systemic hormone therapy before age 60 or within 10 years of menopause have a favorable benefit–risk profile for hot flashes/night sweats and bone protection. Risks depend on type, dose, route, and timing; we’ll personalize and reassess regularly. Seek urgent care for chest pain, trouble breathing, sudden severe headache, one-sided weakness, vision changes, or heavy unexplained vaginal bleeding.

FAQs

Are pellets safer than patches or pills?

Not necessarily. Pellets are usually compounded and lack the same approval and dosing consistency as FDA-approved products. They also can’t be quickly removed. Many women do well with approved transdermal (patch/gel/spray) or oral options.

Do bioidentical hormones mean “risk-free”?

No. “Bioidentical” refers to hormone structure, not risk. Benefits and risks depend on dose, route, and timing. We use the lowest effective dose and reassess.

If I have a uterus, do I need progesterone with estrogen?

Usually yes, to protect the uterine lining from overgrowth. Women without a uterus typically don’t need a progestogen.

What problems have been reported with pellets?

Reports include variable dosing, difficulty adjusting or stopping, and occasional site issues (infection/extrusion). Some adverse events have been reported to the FDA, underscoring the need for careful selection and monitoring.

Who should avoid systemic hormone therapy?

Women with certain cancers, unexplained vaginal bleeding, active or high-risk blood clots, stroke, or significant liver disease may not be candidates. We’ll review your history and coordinate with your clinicians.

How soon might I feel better?

Many women notice fewer hot flashes and better sleep within a few weeks; dose-tuning can take a bit longer. We’ll adjust based on symptoms and goals.
